Add a hazard
A hazard is anything that could cause harm. Adding it to the register is the first step toward managing it. This guide assumes you already understand what a hazard is — if not, see Hazards, incidents, and near-misses.
- From the sidebar, open Hazards.
- Click + Hazard at the top right. The hazard wizard opens.
- On the Basics tab, fill in:
- Title — short, plain-language summary.
- Description — what the hazard is and where it shows up.
- Site — pick from the dropdown.
- Location — free-text sub-location within the site.
- Type — pick from the lookup, or leave blank.
- On the Assess tab, set inherent likelihood and severity. (See Assess inherent and residual risk.)
- On the Control tab, list controls already in place and assess residual risk. (See Add controls.)
- On the Assign tab, pick a responsible person.
- Optionally add photos on the Attachments tab.
- Click Save.
You land on the new hazard’s detail page. The BRAG colour reflects inherent risk, residual risk, and review-date proximity.
Bare minimum
Section titled “Bare minimum”If you’re rushing to get a hazard captured during a walkthrough, the fields you must complete are:
- Title
- Site
- Inherent likelihood
- Inherent severity
Everything else can be filled in later. The hazard will show as red or amber on the dashboard until it’s properly assessed and controlled.
